PERIOPERATIVE SURGICAL HOME SERVICE:

Perioperative Surgical Home is an innovative model of delivering health care during the entire patient surgical/procedural experience; from the time of the decision for surgery until patient recovery. It is designed to achieve the triple aim of improving health, improving the delivery of healthcare, and reducing the cost of care. These goals will be met through shared decision-making and seamless continuity of care for the surgical patient, from the decision for surgery through recovery, discharge, and beyond. Each patient will receive the right care, at the right place, and the right time.

Perioperative Surgical Home Initiative at St Francis Medical Center:
Together with our surgical colleagues, Landmark anesthesiologists at St Francis Medical Center will work to develop and apply evidence-based, outcome-oriented protocols for the perioperative care of our surgical patients. Enhanced Recovery After Surgery (ERAS) recommendations will be utilized to establish standardized patient care pathways to ensure high quality care and improved outcomes for the surgical population. The initial initiative will be focusing on two key areas, orthopedic total joint replacements and colorectal surgery. A multidisciplinary task force led by Dr. Hanh Nguyen-Clark will formulate ERAS adapted protocols for optimized and standardized multimodal perioperative care. Along with Dr. David Friedberg (orthopedic total joint replacement surgeon) and Dr. Jose Montano (colorectal surgeon) the team will head up the implementation of these processes into the daily clinical practice and the education of all team members regarding these methodologies.

TRAUMA SERVICE:

The St Francis Medical Center Trauma Center is a certified adult Level Two trauma center in Lynwood. Our trauma facility is responsible for the care of more than 2,000 cases per year with a multidisciplinary team made of trained resuscitation specialists that includes an anesthesiologist, a trauma surgeon, a neurosurgeon, a trauma nurse, and paramedical personnel. Other specialists such as vascular surgeons and orthopedic surgeons are also involved in the care of the trauma victims.

The center oversees the care of all trauma victims in South Los Angeles. Our annual trauma volume for 2016 was 2,418 patients. 762 patients (32%) were Tier I. Blunt trauma was about 73% and penetrating overall was about 26% (Some patients can have both mechanisms and often times on weekends the penetrating rate can be the majority). Blunt trauma is typically from motor vehicle accidents, fall or industrial accidents, while the remaining 30 percent of the admissions include gunshot wounds, stabbings, and burns. All patients receive the most advanced treatment regardless of their ability to pay, making the SFMC Trauma Center the centerpiece of South Los Angeles County’s health system.

The trauma anesthesiologist has a key role in the successful resuscitation of the injured patient. The anesthesiologist is trained to give necessary critical care (e.g. advanced airway management, shock resuscitation, and interpretation of invasive monitoring) and can perform these functions continuously from arrival to the trauma center through the operating room and recovery units including the intensive care unit.
